Colorectal cancer screening rates are suboptimal, prompting provider organizations to consider creative ways to motivate patients and eliminate barriers to screening. In a 2017 pilot, three Southern California health care practices improved screening rates using:
•Population health management techniques for patient identification
•Outreach tools built on an understanding of patient preferences, and
•A series of internal workflow changes

THE CHALLENGE:
Colorectal cancer screening rates lag behind national goals — at odds with today’s emphasis on prevention and quality of care. Health care providers and systems have incentives to encourage screenings, but quite simply, many patients are difficult to engage. As the lead editor of this case study, I worked with a health system to understand its strategy for patient engagement, distill best practices that may be instructive for peer organizations, and help its leaders communicate outcomes through this summary article and an original research paper.
